  • A surgeon performing a colostomy procedure on a patient and tied the wrong end of the bag. Fecal matter seeped inside the patient, causing sepsis (blood poisoning) and eventually the woman’s death.
  • During treatment for an infection, X-rays of a patient revealed early stage cancer. Through tragic bungling, the man was not informed until seven months later that he had cancer  by then the disease had incurably spread.
  • A podiatrist used the wrong surgical screws in repairing a broken foot. The screws were too small and came out of their settings, resulting in severe pain and incorrect healing. The client is now disabled, and has since endured four more surgeries.
